Hello Steven,
How exactly did you try to install this extension? Please provide a step-by-step explanation.

Hi Teodor,

I use DMXZone Extension Manager.

I opened CC2015 and was confronted with the screen capture you would see if you follow the link.
The popup does not work as it is malformed.

I had to install through Extension Manager all the extensions into CC 2015 (drop down at top of DMXZone Extension Manager).

I expected there may be some extension problems but did not expect this. CC2015 is almost unusable. I know not your problem but something also wrong with the extensions and the installation.

Were the extensions in the "installed" list? Or did you install them?

Not sure. There are quite a few that popup when I open Dreamweaver.
I did install quite a few in extension manager as when I dropped down the version at the top the number of installed extensions changes.
Is there a known good list of extensions for CC 2015?

Why would that cause the form to become corrupted?

Please do the following:
1. Close DW and open DMXzone Extension Manager
2. Select DW CC2015 and click the "Installed" button
3. Remove ALL of the extensions.
4. Restart DW and check if the ugly activation popups appear again?
IF NO:

5. Close DW and open DMXzone Extension Manager
6. Click the "Explore" button and install several random extensions (4 or 5) Ignore any warnings about the versions incompatibility and select install anyway.
7. Restart DW and check if the ugly activation popups appear again?

If NO:
8. Install the rest of the extensions.

Hi Teodor,
Some extensions won't delete and come up with an unlink error. CC 2015 crashes now.
It is 1:06AM here so will have another look in the morning. Any advice will be gratefully accepted.

What exactly do you mean by: "Some extensions won't delete and come up with an unlink error. " ?!
Please reinstall your CC2015 and start again...
