Is it possible with this or a combination of extensions to accomplish the following:

From a single form --I have TWO image upload fields. One for a high res and one for thumbnails. There are also fields for description, catalog number, etc. All the information eventually ends up in a database.

So I would like to have the high res version go into the HIGH RES folder and the thumbnail go into the THUMBS folder and the text data go into the database.

If you use one upload field and you use together with the Pure Upload Smart Image Processor you will be able to set different folder fore the thumbnail image that will be produced from SIP. Check the Smart Image Processor manual for more information on how to get both extensions working together.
