PHP Upload 2 Extension Error

Make sure there is a comment at the top of the file in this format <!-- search:"string to find" --> Example: <!- search:"rs.Request(\"r" --> ------------------------ I know I am making assumptions, but, I thought I would just insert this extension into a page, fill out the variables, and have a functioning upload process. I am not sure what the comments are for or where they go.
