Shopping Cart not completed

I followed the book E-Commerce  tutorial and found out that the Shopping cart missing few pages in order finish the shopping cart, The author Did mention in his books how you can control the shipping cost, he even created a table called “Shipping Methods” that contain different shipping options such as UPS Ground, 2nd Day Delivery and Overnight but never bother to cover that, so for those who needed to have Shipping added to their cart good luck you wont find it in this book, I have tired to contact the author several time but with no success, he did not acknowledge my request,
