dynamic form probelms in dreamweaver mx

Hi:

I am pretty new with using dreamweaver, mysql and php, and am thinking my problem is simple.  I am trying to create a form based on a dynamic table.  The table has 3 columns- ID# , date submitted, and description.  What I'm trying to do is to add a radio button to each row that will send the ID #   into a mailer form I built.  Along with the ID # is info the user fills out in the form (name, email address etc).  I've created the recordset and the dynamic table no problem, and when the form is completed, it mails all the info except the ID# correctly.  Was wondering if somebody could give me an assist on this. 

Don't know if this is relevant or not, but I created the form, then created the dynamic form table inside the form box, then created another column for the radio buton and used the dynamic button to bind the ID value to the radio button.  Also, I had read somewhere that I was supposed to create a radiobutton group, but since the dynamic table only shows one row when I'm creating the form, that didn't work. I clicked on the single radio button instead and bound it to the server setting.  Any help with this would be very appreciated
