Hello Brad,
Hot spot ( or other way named mapped region overlaying an image ) has some limitations when it comes for determine its width and height. Those difficulties are the cause for tool-tip to be not able to exact find its location.
Setting the tooltip to show next to mouse cursor may fix this.
