Can you tell me how to add a <textarea> to the shopping cart as well as a <select> option? I have an example cart set up at www.carolinawreath.com/wreath.php?ID=10 which successfully collects the select data, but not the <textarea>

Have been hoping tech support would be open today, but looks like a second day you are offline. This is very time sensitive so hoping you can come back quickly in the forum.

Any additional options that need to be passes, should be presented as dropdowns with choices. There is a tutorial how to do this.

I have radio buttons on the form that are working well. But I need the customer to be able to give additional instructions through a text field. Is there a way to do that?

I already explained you that this is not possible. It is not included in the extension's features.

Teodor, I really like DMX, you and Miro have been wonderful and very very helpful. I find your tech support great, and on the whole your extensions are wonderful, but this extension is useless to me Is there any way to pass even a text field that allows customer entry without giving specific set options?

All of the features, included in the extension are described in the features section on the product page, as well as in the Video Tutorials and User's Manual.
If a feature is not described there, then it is not included in the extension.
