.htaccess causing problems

We have four areas that we need to have people upload to. We have created the four folders under an ftp folder, where they choose which area they are going to upload to. On choosing the area, they are presented with a login created in .htaccess for that folder. Once they enter the username and password, they are taken to the index.php, where the pure php form is installed. In testing, we find that after browsing for the file to upload, and starting the upload, there is a problem. First, when the upload is completed, you are asked for the username and password again. Then, it attempts to upload the file once more. When that reaches 100%, we get a failure because of insufficient permissions. Can this not be used in conjunction with a password protected folder?

OK. Since no one chimes in, we've cut it back to one upload area, and the permissions problem seemed to be fixed. However, testing with Macs, they are unable to upload. Get the same lack of permissions problem. HELP!!!!!
