I have installed Pure PHP Upload in Dreamweaver 5.5. I got the install wizard and it completed without issue. I see the extension enabled in extension manager. I tried to follow the tutorial but cannot find any DMXZone or PHP Upload icon to choose to insert PHP Upload. I have a new page with form, file field, and submit button. I see Common, Layout, Forms, etc., all the way to Favorites but no DMXZone.
