Hello Alan.
I will suggest you the following troubleshooting:
1.)Uninstall the extension from EM.
2.)Close DW.
3.)Clear your DW cache file.
4.)Download fresh copy of the extension from the product page.
5.)Install the extension in EM
6.)Restart DW
7.)Proceed the install shield in DW.
8.)Restart DW when the install shield in DW is completed.
9.)Try to place the extension again on blank page.
