Could you please be a little more detailed explaining step by step what exactly happens? Also - which extension manager are you using and what do you mean by "empty icon"?

I've tried it with both the DMXzone and the Adobe Extension Manager. Same results. The Adobe one shows it to be installed and the DMXzzone one does not. The icon is there but grayed out with a question mark in it and it's also in the insert menu. Nothing happens at all clicking on either. I just recently went to Dreamweaver CC 2014 with my subscription. The original CC is fine with all my DMXzone extensions. The Extension Manager, slider and Nav Menu installed OK with CC2014 but the others did not, including the Map.

Please uninstall all of the extensions from Adobe Extension Manager CC 2014 and try reinstalling them using the DMXzone Extension manager.

Did that but still have the icons with ? and the items in the insert menu. Please help me get everything out of Dreamweaver then I'll start all over. There seems to be no way to remove anything in the DMXzone Ext. mgr. but the ones I'm concerned about are not there anyway and the icons and menu items are still there. I need to get EVERYTHING DMXzone out then I can try starting all over again. Otherwise I'll just have to consider DMXzone a loss and put up with the ugly ? icons that do nothing.

Please removea any extension installed using ADOBE extension manager, by starting ADOBE Extension Manager and clicking the Remove buttons there.
Then just start DMXzone Extension manager, select your Dreamweaver version from the versions menu, click the EXPLORE button an install ANY extension that you want.
The extension should be started from within the Insert Panel, and not the Insert Menu.

Notice my last note said "Did that." Everything has been removed from Adobe Ext. Mgr. They do not show in DMXzone Ext. Mgr. but all the icons and menu items are still there.

Did you go to Explore section of DMXzone Extension manager? There is an INSTALL button for each of the extensions...
Also - please provide a screenshot of what exactly do you see in Dreamweaver.

They do not show as being installed. I realize they show under the install tab. But, the icons with the ? and the menu items are still there. They should not be if the extension is not installed. Let's get rid of those before re-installing the extensions. Then we can do a completely fresh install.
