Appointment Form with Interactive Notifications

If you need some more inspiration when working with HTML5 Responsive Notify 2, here's one more showcase for you. We used a few awesome DMXzone extensions to create an appointment form. It is fairly easy to make with the DMXzone Ajax Form, which we connected with the HTML5 Responsive Notify 2 to inform our users when they submit something successfully. It is also possible to choose a date and time for the appointment with the DMXzone Calendar 3 and DMXzone Timepicker tools.
