Insert Record With Ident errors

Hi,I downloaded the extension and installed it on DWMX. Since then I get the following error code when I try to use this extension or the built-in Insert Record extension:
At line 927 of file "C:\Program Files\...EditOperation.js": TypeError: tagList[i].getAttribute("NAME"
has no properties.I disabled the extension and deleted it but I can no longer use the Insert Record behavior now.
Can someone please help?
