does anyone know of a cgi, php or dreamweaver extension for sending of a email form from a site like this.

I've used loads of cgi scripts before for sending emails , but im trying to find one that if a field is left empty, the email won't include the field name or value in the email
for example if I had a page that had name, address email and address wasn't filled in then normally with the scripts I have used the email would look like this

name: john doe
Address:
email:

what I want it to look like is this:

name: john doe
email:

so it completley misses out the address

can anyone help??? it's pretty urgent
