PHP Upload

When I use the php upload extension in a form and try to close and the extension it tells me: "While executing applyServerBehavior in PurePHPUpload a Javascript error occurred." I don't have the popup stopper on and not sure how to fix the error so I can use the PurePHPUpload extension. Any ideas?

A simple form works but the form I am currently working with fails. It has a template with uneditable regions. Would this be a problem?

Normally this should be no problem. Does the error also refer to a line number where the error occurred, or is that the exact error.

I figure it out. In the form I am page I am working in I had a form that was unnamed and this was where I was inserting the file upload. The extension crashes with an unnamed form.
