help in getting google toolbar autofill my form

I have a form created with dreamweaver on www.morgle.com/mailregtestfieldsI cannot get the google toolbar to autofill the adddress fields.
I ahve looked at the google help referring me to www.ietf.org/rfc/rfc3106.txt
buit no matter how I name the fields I can't seem to get them recognised as autofill.
Can anyone assist please

thanks for info. I want the forms to fill in automatically because it is part of an ecommerce site and it saves people entering details if they use the google autofill facility
